 lk about...leaving hospital\" - an open meeting \nThis event will exp
 lore the issues around older people leaving hospital and the support a
 vailable to them at home or in the community once they have been disch
 arged.\nYou will be able to find out more about local services which p
 rovide at home or community support to older people (over 65s) and peo
 ple with long term health conditions, once they have left hospital. Pl
 us you’ll have the chance to share your thoughts on the topic, inclu
