Hospital Patient Transport is subject to eligibility criteria. Hospital Patient Transport will not be provided for anyone who lives within six miles of the North Middlsex hospital and is under the age of 65, unless the patient meets the eligibility criteria, and this also applies to any follow up appointments.Importantly, Hospital Patient Transport will NOT normally provide transport for a person accompanying the patient UNLESS IT HAS BEEN PREARRANGED with the clinical teamDial A Ride Members are NOT intended to use this service to attend hospital for hospital appointments but Dial a Ride can be legitimately used for visiting a patient as the booking time for dial a ride is not considered to be time critical
Patients should use the cheapest most appropriate means of transport, which in most cases will be public transport. If you travel by car you will be reimbursed the estimated cost of fuel used plus unavoidable car parking and toll charges.
Where you are using a taxi for transport it is recommended that you check with the hospital orClinical Commissioning Group (CCG) before you travel to agree this.
